Community Engagement for Developmental Disabilities

Individualized Planning

Our team develops an Individualized Service Plan (ISP) based on the assessment, with clear and measurable goals focused on daily functioning, independence, and stability. The plan is built around what matters most to the person and reviewed regularly as progress is made.

Community-Based Service Delivery

Services are delivered in homes, in the community, and in the everyday settings where life happens. This allows people to practice and reinforce skills in the same places they will actually use them, which makes progress more lasting and more meaningful.

Ongoing Monitoring
